By scheduling an appointment or purchasing consultation services from PharmAssist Weight Management and Men’s Health, you acknowledge and agree to the following terms and policies.
Consultation Fees & Refund Policy
A $250 non-refundable consultation fee is required for all new patient weight loss consultations.
A $250 non-refundable consultation fee is required for all new patient hormone therapy consultations.
Consultation fees are non-refundable because professional time is reserved specifically for each scheduled consultation.
Payment for a consultation does not guarantee any specific treatment or outcome. Recommendations are determined following an appropriate professional evaluation.
Payment information must be provided prior to the scheduled appointment. Failure to provide required payment information may result in cancellation or rescheduling of the appointment.
Cancellation & No-Show Policy
PharmAssist requires at least 48 hours’ notice to cancel or reschedule an appointment.
Cancellations made less than 48 hours before the scheduled appointment are subject to a $50 cancellation fee.
Cancellations made less than 24 hours before the scheduled appointment, as well as missed appointments/no-shows, are subject to a $100 fee.
Patients arriving more than 15 minutes late may be required to reschedule and may be assessed the applicable cancellation or no-show fee.
Outstanding cancellation or no-show fees must be paid before another appointment can be scheduled.
Payment Authorization
By providing a credit or debit card, you authorize PharmAssist and its payment-processing provider to charge the payment method provided for consultation fees and any applicable cancellation or no-show fees described in this policy.
You agree to provide accurate and current billing information.
Billing Descriptor
Charges from PharmAssist will appear on your credit or debit card statement as:
