Weight Loss Consultation
A non-refundable consultation fee of $150 is required for all weight loss consultations.
If payment information is not provided in the required intake paperwork, our office will attempt to contact you prior to the appointment to obtain a credit or debit card on file. Failure to provide payment information or failure to respond to outreach prior to the scheduled appointment may result in appointment cancellation or denial of service at the time of the appointment.
Hormone Therapy Consultation
Hormone therapy consultations require a non-refundable consultation fee of $250. Failure to provide payment information prior to the appointment may result in cancellation or rescheduling of the consultation.
